Job Duties: Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem. Interviews patients and family members. Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations. Participates in conferences on behalf of patients. Provides service planned to restore patient to optimum social and health adjustment within patient's capacity. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability. Evaluates patients regarding social problems, arranges discharge and post-operative care including nursing home placement, provides assessment and referral in child and adult abuse cases, refers to Bureau of Social Services, and assists with adoptions. Prepares patient histories, psychosocial assessments, service plans, and documents in the medical record. Participates in educational programs. Assists department staff in locating nursing home beds for KDMC patients and may function as a nursing home specialist. Prepares statistical reports and participates in quality improvement studies.
Qualifications: Bachelor''s degree (B.S.W.) from a four-year college or university; Master''s degree preferred and Kentucky licensure is required. 1-2 years related health care experience preferred.
Physical Demands: While performing the duties of this job the team member is frequently required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms: climb or balance; stoop, kneel, crouch, or crawl; talk or hear and taste or smell. The team member must regularly lift and or move up to 25 pounds, and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
