JOB SUMMARYThe Medication History Technician will work primarily in the Emergency Department to assist the pharmacy, nursing and medical staff in obtaining accurate home medication lists from patients entering the hospital and conveying that information verbally or electronically to the appropriate provider of care.
The Medication History Technician may also work in Day Surgery, Pre admission testing and on the inpatient nursing units.
DUTIES AND ESSENTIAL FUNCTIONSUtilizes a standard process of interview and phone calls to retail pharmacies and physician offices, and may use medication history software and state databases to gather home medication historiesUpdate computerized home medication data bases and reconcile these lists to orders written upon admission to the hospital.
Discuss select high alert/targeted medications with the pharmacist, as appropriate.
If the medical history was updated AFTER the provider medication reconciliation, notify the pharmacist immediately of any discrepancies between the home medication list and the admission orders.
Collects data, conducts quality monitors, and completes documentation as assigned.
EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTSMinimum requirement:
High school diploma or GED Certified Pharmacy Technician (CPhT) required or current enrollment in a college of pharmacy (intern)Active Kentucky Board of Pharmacy Technician registrationPreferred
Qualifications:
Two years of previous pharmacy technician experience (1-2 years of retail experience considered positive)WORKING ENVIRONMENTConstantly works near moving mechanical parts Constantly exposed toblood and/or body fluids andinfection.
Occasionally exposed to toxic or caustic chemicalsOccasionally exposed to risk of radiationThe noise level in the work environment is usually moderate.
